I have found a specimen of 3632A with a die cutting issue. Two of the valleys on the right side of the stamp were not cut. Thus, when the stamps were separated, it tore the paper like an old-style perforation.
Would this have been caused by a dull blade? Other theories?

Opinion. Yes I agree, it surprises me there is not more of this, For the amount being die cut, married to the very small margin of error. An amazing record for modern technology.
I see that it actually affects two valleys as mentioned. Is it possible the cutting edge was hit by something and bent the blade/die up and away from the normal height.
I've seen/handled a die cutting die at the BEP - fairly shallow, but if hit hard enough, sure part of it could be deformed enough to no longer come into contact with the stamp paper.
I've also seen coils that were poorly die cut along the entire height, but it is unclear of ot was a worn die or low pressure.
