I've been wanting one of these for some time. Found this on eBay; not hard to find, or very expensive.

Was interesting to me for several reasons:
1. Neat & clean.
2. Clear cancel (US Navy 1944)
3. Able to associate to ship and location (USS PC-571 - Patrolling Hawaiian Islands)
4. Interesting hand stamp. (Applicant is NOT registered)

Here is a state-produced ballot request form:



The county clerk then mailed the ballot out to the soldier:


The soldier mailed it back in a slightly smaller envelope with their certification on the back side (this one trimmed some at right):



And a privately-made ballot envelope! I believe this is quite uncommon.
