What would the TEST be for?
No back stamps.
The black hand stamp above Baltimore & Ohio top left is June 30 1920. It appears the cover was returned to the original sender per the content letter signed by Henry Koenig.

My first guess would be the B & O railroad company tried to find out how long a letter takes from A to B...

It is a well known private file marking from the recipient Brooks company. It stands for "testimonial." The company sought and received customer testimonials for the use of its products. The covers are widespread in the market.

Drkohler, that's what I was leaning into but sure enough paper history I believe has nailed it.

The letter enclosed from Henry Koenig is praising something that he had purchased, a 'truss' maybe, I can't decipher the hand writing. I just took a second to google Brooks and sure enough they sold belts/trusses for people suffering from hernias.

Now another look at the cover I can see its addressed to Brooks Rupture Appliance Co. and it makes sense.
