Someone was obsessed with George Washington. 3 different stamps representing different ages of the President, and a cachet (which I cannot tell if it is contemporary or not).
Clearly the creator was having fun creating a one-of-a-kind piece. A lot more interesting than a mass-produced PCS cover that would have cost the owner 10 times what it cost him to create this one.
Hideaki Nakano made a name for himself creating such FDC for several decades until his death. Many were combination covers build on related older covers (often FDCs) or a series of different FD stamps on one cover over a period of years. Search his full name or just his last name on ebay to see the breadth of his wonderful work.
