Australian KGV - Line Or Comb Perf?

This looks to me like a badly executed line perf job, but I'm certainly no expert. What do people think? Line perf or comb perf? In this case, there's a rather large difference in CV.

SINGLE LINE PERFORATION
The single line machine gauged 14.2, the comb machine 14.25 x 14. Thus the side of a
single line perforated stamp differs appreciably from the side of one comb perforated and
it is quite simple, bearing this in mind, to distinguish one perforation from the other.
To do this, take the stamp thought to be single line perforated and place alongside it a
stamp known to be comb perforated (say a 1918 or any rough paper issue), letting the top
perf. at the side of each coincide. It will be seen that the difference between the
perforations becomes progressively greater down the sides of the stamps, finally
becoming obvious to the naked eye, without recourse to perf. measuring apparatus.
The specification for both machines was no doubt '18 holes to the inch'. The closest
approximation to this is provided by the 14.2 gauge of the single line machine.

Speaking of KGV varieties, I just found this one this evening. I had actually thought it was a tiny sliver of white paper stuck to the front of the stamp, but when I checked I found it wasn't. It's a pretty conspicuous gash, so, I would assume, it's been noticed by someone before me and probably listed. If anyone (hi, Aussie Al!) happens to know what it is (incl. CV), please honk!

I guess it's not a plate error, then. Presumably, there WAS a little piece of paper on the plate when the stamp was printed.

It is classified as an EFO.
When / If you find another, it becomes a plate flaw.
