My guess would be a nurse nic but you're absolutely correct it could be both.

A lot of nurses back then were both. Many of the hospitals were run by the catholic church. That being said there was also the belief that, just like in food service, things were cleaner if your hair was covered up.

I think your right dianne and ziggy9 a nun would probably be placed in a prayerful pose with a halo overhead, but I dont see any stethoscope? or ballpens?
