It looks like a wheel pair got loose together on the outer right side since the wheels would have had more room to move around there and these stamps show large outer margins.
I don't think it is possible for the perforating wheel to move that much without separating the perfs or ripping the paper in the process. I am more in favor of the theory that the paper developed a bit of a pucker as it went through the perforating machine.
