The round ones (like this) are usually called pumpkin heads or jack o' lanterns. The irregular/non-circular shapes are usually called masks. There's no set naming rule, really.
This one is not listed in the Herst-Sampson book, at least.
Peter - no idea. It was already soaked off and just sitting on a page that a dealer friend had with other fancies. All I can tell you is it has a grill (presumably a 94, I haven't checked), so it is not a 65, which helps date it some.
