It finally struck me, I can see the relevance finally.

Mercury is riding front, and right hand side of a locomotive
"The weird blobs" are of a locomotive lantern.
Part of the boiler, is behind Mercury's winged helmet.

Partnering maybe LARUNDA, Mercury's consort.

Likely 1924. I found the general elections for the 27th legislature (Kingdom of Italy) took place on 6 April 1924. Maybe 27a was a second round for undecided districts.

Requires further research, but maybe this was either on the document inviting to vote, or applied to a document once voted.

Not sure whether this lines up with info provided but this part of the inscription is legible in below picture.

I am puzzled by the classification 'postal fiscal.' I doubt it had any postal use and would be a ' fiscal.'

Agree not postal or fiscal
The good thing, we now have a catalogue reference "Unificato"

I'd suggest perhaps a charity based promotional stamp. (But a Crown watermark?) may suggest otherwise.

As your site suggests: "hotel bills."

It, likely, is a stamp affixed to the hotel bill kept by the hotel showing the amount of tourist tax received.
