P P C With Improper Postage

I picked this one up for the interesting stagecoach scene, but as I was examining the address side I quickly noticed a discrepancy. The card was mailed to England, which should have required a 2¢ stamp. In fact, it even has "Foreign 2¢" peeking out below the stamp! I believe this has a 1911 postmark, though it's hard to read. How do you suppose it escaped without postage due?

1. Human error.
2. Never went to England. After cancelling, error noted and card handed back to sender without any docketing. Sender never added additional postage and offered for remailing.
3. 1 Cent stamp did not originate on this card it replaced a removed 2 cent.

Human error seems the most likely to me. I don't see how it could have gotten back to the sender and I'm quite certain the original stamp remains in place.
