This was in a recent purchase that was just at the bottom of a box. Thus the wrinkled state. It has stamps similar to B12 etc. but the amount of the surcharge is different. IE: B12 should have a 1˘ plus 2˘ and this is a +5˘ on the 1˘. This holds throughout the 5 stamps. Any info appreciated.

Michel Block 14A,issued in 1958
It can be found imperforate (Michel Block 14B) and with URNWA on the right (Michel Block 13A)

Your miniature sheet with the +5c surcharge is noted in Scott after B20, but not listed for some reason. The miniature sheet itself is noted after Scott 488, saying it exists with no mention of being "issued".
