I am fairly certain that this stamp is 1622C with 11.2 perforations and printed on Combination Press. Any information on this type of error is greatly appreciated. I did not find a similar topic when searching this forum.
Such misperforations are known on the issues with varying amount of shifted perforations. They should have been caught as waste,removed and destroyed but all were not. Yes a production error but in terms of EFOs, the term of art is Freak, not an error.

EFO= Errors, Freaks, and Oddities, if I remember correctly. A very broad category.
