Looks like Scott 632 to me [11x10.5 perfs, tall shape, yellow green]. It is impossible to tell conclusively from a scan, but the side perfs look different from the top/bottom. It is not wide-shaped [578 & 594]. The colour doesn't look like 552.... etc...
Do you have a perf guage at your disposal? The size of the perforations is the first thing you'll need to check in order to determine type. From the scan, I can tell you it is not a perf 10 variety, so 578 is out. If you are unsure what a perf guage is, we'd be more than happy to help you with that ...
