#68 Perforation Issue: Tear?

I'm wondering if any of the resident experts have ever seen this sort of issue. #68 pretty well centered, but in the UR corner there is some sort of damage or perforation problem. It's not the right location for the next perf, but appears to be semicircular and "perf-like". There's a piece of the stamp hanging by a few threads there too; it is twisted around with the back side to the front (the back of this piece as it stands now more matches the front of the stamp).

Is it just plain damaged, or was there maybe some sort of fault that happened during the perforation process?

A possible reperf across the top? Still a great looking stamp IMO.
