Bottom Row, Third Stamp (US/F&G) = United States Fidelity and Guaranty Company.
Bottom Row, Fourth Stamp (UP) = Union Pacific Railroad (looks like a double perfin).

A bit slow with my reply, oh well.

Row 1 stamp 1 - NPB - The National Park Bank of New York, New York NY - perfin pattern N131, rarity factor E

Row 1 stamp 2 - DP(H) - Department of Health, Harrisburg PA - perfin pattern D88A, rarity factor F - one of two very similar perfins

Row 1 stamp 3 - UN - The University of Nebraska, Lincoln NE - perfin pattern U43, rarity factor E

Row 1 stamp 4 - C(Co) - Cook County, Chicago IL - perfin pattern C83, rarity factor E - similar designs exist for The Centaur Company and The Century Company, both from New York NY

Row 2 stamp 1 - CNB - Chemical National Bank, New York NY - perfin pattern C211, rarity factor E - there is a similar 5 pin high CNB perfin for the Chase National Bank in Washington DC

Row 2 stamp 2 - I/WILL - City Of Chicago, Chicago IL - perfin pattern I117, rarity factor F - one of 3 very similar perfin patterns differing in the shape of the W, along with a 4th similar pattern that is vertical

Row 2 stamp 3 - US/F&G - United States Fidelity & Guaranty Company, New York NY - perfin pattern U75.3, rarity factor F - there are 5 similar 4 pin high US/F&G patterns, two of which have the extra control pin at the bottom of the ampersand, but that apparently didn't refer to an office location (the other one with the extra pin is supposedly from the Baltimore MD office)

Row 2 stamp 4 - UP - Union Pacific Railroad Company, General Offices Omaha NE - perfin pattern U59, rarity factor F - there is another perfin pattern U59.5R which is identical expect for a bunch of missing pins, and it looks like your copy is well on its way to having a bunch of dead pins!

Row 2 stamp 5 - CNB - Chase National Bank, New York NY - perfin pattern C219, rarity factor F - one of 6 similar 5 pin high perfins, this is the only one with the control pin below the "C"

Row 2 stamp 6 - MNB - Mercantile National Bank of San Francisco, San Francisco CA - perfin pattern M184, rarity factor F - there is a very similar (but very rare) MNB perfin for The Merchants National Bank of Providence RI

Ryan: Great information that you share on identifying these perfins.

Since you seem to have the catalog that answers a lot of these perfin questions, would you care to help identify some additional perfins that I have that start with the letter "C"?
