I doubt that it is an error. For it to be an error there have to be a whole bunch of them. I think this is an interesting freak or oddity!

This appears to be an ink void, likely caused by a small piece of something (paper?) stuck to the stamp paper prior to printing. The ink was applied to this and then flaked off post-printing.
