From my examples, he seems to have always had an issue under his proboscis. Almost looks like part of a hitler moustache or an "M", so perhaps just an engraving oddity. Your example seems to be rather a smudge.
A similar oddity is the left eye of qe2 in the GB wilding engravings. Has varying engravings as dark spots.
Small variances such as that are common in engraved stamps. The plates are typically laid down with a multiple relief transfer roll. This is probably a releif variation where the plate positions of the two stamps in question were probably produced by different reliefs. Most plate are made from 3-5 different reliefs.
