I have search the Internet with no positive results associated with this envelope. Can anyone help identify it? Is it a USPS item (no results from searches) or Western Union or some other entity?

Not really having a clue (but you peaked my interest), I tried googling the form numbers. What I came up with are some hits regarding military mail consolidated reports. Form 2755, for example, comes up as:

Not sure if this is the right answer or not, but it may provide you a lead for more sniffing around the internet.

Looks like a cover intended for submitting a completed form of some sort. Government usage probably. Bethcha dates around 1950.

Different color paper stock and slightly different wording but similar Form Numbers. So if this auction item is any indication, the Mail-O-Gram was a product of Canada dating back to at least the late 1950s (in this case 1959):
