How does Steiner compare with the Scott National in terms of pages for US revenues? Are there options beyond those two that I should also consider.

Yes,
but from what I can see, US only.

Why do I get a dotted line around the image?

Palo makes hingeless pages for all Scott-listed U.S. revenues, i.e., the pages are complete as far as all Scott major number listings. I have the Stock Transfer pages.

@ROD -- the dotted line means that image will open in a new window when clicked. It happens when images are especially wide or tall.
