The little lines of color above the NARC suggest it was applied by a office supply stamp where you can rotate the letters to stamp out the word you want to imprint. Real narcotic handstamps would not have been applied this way.

Yes there was, although it was later as a large green strip stamp. The year date is much too late for a provisional, 193(something).

I agree that the late date in the 1930's is the first problem. The name of the canceller doesn't match the pharmaceutical companies we know. The only reason for such a high denomination is for import and again this name doesn't seem to match any of the known importers. Finally, as has been said, we don't know that a $4 was ever created. The high denomination provisionals were used in 1919 in New York and Indianapolis. There may have been other ports of entry, but none have been documented .
