Apparently, the United Postal Stationery Society (UPSS) has a book about them.

As Trainwreck noted above, the UPSS 2013 Specimen catalog lists the U621 samples. These samples were given away over the counter at numerous post offices. The original printing has the higher value and was issued in regular and legal size, both on lite blue paper. The sample reprint has a much lower value and was also issued in regular and legal size, both on lite blue paper. The item depicted is the small size (12) reprint valued at $12 in the UPSS catalog.

Question: Is that postmark inappropriately applied, since sample envelopes are invalid for postage?

I suspect a clerk may have overlooked/misunderstood the invalidating word "sample" and thought it was still "accountable paper" and needed an actual cancel before serving its advertising purpose. The postmark is unnecessary.
