I hope this is not too detailed.

The lamp I got also had a multi-page book on minerals and their luminescent properties.

I just bought an old working LS-4 Raytech UV light on ebay recently for less than $50, the seller stated that the old bulb was frosted over and he had replaced it with a Philips germicidal fluorescent tube? which I think seems to work the same.

After receiving the LS-4 in the mail (it came in its original box with the paperwork, the items original worn out bulb, and some mineral samples) I plugged it in and it vibrates like hell but it runs well and it includes the sliding filter screen. Both the sliding screen and replacement bulbs I think could be difficult to find but I haven't tried looking for them.

With the sliding screen, from what I understand, you need it so one can 'slide' from short-wave to long-wave to filter out one or the other wavelengths.

Yes, the slider is intended to block one side. I do not think the notch filters as good (meaning not a narrow wavelength as some SW lamps) but generally works. I have 2 LS-4's I was using for even lighting for photography.

Not having ever seen a unit, are they really notch filters?

You can always search alibaba for zwb1 and zwb3 filter glasses.
zwb1 in front of the lw uv bulp kills any short wave uv (stray light from the other tube?) and visibe light (which is also generated by a lw bulp).
zwb3 in front of the sw uv bulp will kill some of the uv sw light and most of the visible light (which, again, is also generated by a sw uv bulp).

I built my own uv lamps using zwb3 glass on the 254nm tube and no glass on the lw tube (which does not seem to emit perceivable visual light).
I also use a Hoya Fusion Next filter in case the detector is sensitive to uv light (my digital miscroscope freaks out when even a hint of uv light is around, the Canon R50 + 85mm macro lense block any uv light fortunately)
