The 2019 edition of the ACSC states "A used example with perforations misplaced 5mm to the right is known". The stamp illustrated in the ACSC is a single 3d brown KGVI with the left side imperforated. The vertical pair above is the MUH example of the same stamp with the 5mm displacement but with both sides imperforate, it is unrecorded and is considered rare.

Lovely oddity. In your opinion how did both sides imperf' come about ?.

What I do know is that the trimmed edges were likely from the sheets being miscut as a result of the misplacement of the perforations, and the reason behind the single imperforation (listed in the ACSC as number 196bb) and double imperforations (unlisted with the same error) would be due to the position of the stamps on the sheet.
