Unfortunately, fake and favour cancels are common on the many Italian stamps - particularly from the 1920s to the 1950s - where the used value is considerably more than mint - for this stamp (Sc 576), Scott is $20 mint, $500 used.
I'm certainly no expert in Italian period cancels, but useful indicators are cancels that are too neat, fit beautifully in a corner, look perfect, etc. - just what you have here.
