The "Booklet Panes and Covers" section in the back of the Scott U.S. Specialized catalog describes the manufacture of this particular booklet - yes, from the same printing layout as sheets, but perforated and cut differently, leaving the left selvage. Thus the remaining selvage/tab adhering the pane to the cover will have various imprints of a plate number, zip code ad, bars, or nothing.
