So I can't read the marking next to the address. What is the reason it wasn't delivered? Or for the additional markings? I see the return address was the same as the mailing address since someone was created a first day cover.

Most often seen on philatelic mail without stuffer card inside.

Two people at the same address, so impossible to tell if it was delivered or returned to sender. Same result.

So envelopes sent for First day cancels would get this treatment if nothing inside?

They *might* get this treatment as the USPOD was covering their behinds to prevent "lost contents" complaints. It is not a very common mark.
