So how did this transpire? Did the sender mail it without postage and the receiving post office held until recipient sent postage? How did he send money or postage? Who put the blue number at the top?

The origin PO held the card and sent a Form 3548.
The "345" is almost certainly applied by Chicago as an internal way of matching the returned stamp with the card.

Thanks John, So the post office in Chicago held it until they received money from recipient then postmarked it.

Almost.
The Chicago PO held the card until they received a *stamp* back, which they applied, postmarked, and sent onward.
