Can anyone on SCF provide some info on these forms.?

Wow I just joined today and saw your post, very interesting.
Looks like you have something that can really be rare as it from 1881.
Below is a link to the Postal National Archives. I would contact them and see what they can tell you.
Your item is mint and I would wear white cotton gloves and store in a acid free envelope with cardboard to protect it from bending.

D&D in old post office notices stands for "Dead and Damaged" mail. It was used to indicate mail that was either damaged or undeliverable and had to be returned to the sender or destroyed1.
