They are listed at the end of the computer vended postage section in the U.S. Specialized. They were from a kiosk ("postal buddy") that was tested in Virginia at the time to print and vend various forms.

Coincidentally (?) a whole bunch of these just showed up today on the Hip Stamp APS store (unused, in sheets of 4). I have not seen these before and now they are turning up everywhere. :-)
