Can someone tell me if this souvenir card was issued with the stamps already affixed to the card in this manner or by someone else after issue. Considering what I see as strange placement of value sequence I can't imagine the postal service doing it as well as mint stamps attached in a lick and stick manner.

Yes this is the order they were arranged and pre-affixed when released. If you Google this string (no quotes), you will see many for sale including with the box that this came in as a souvenir set: 1967 canada centennial souvenir stamp box

Thanks for the reply. It goes with that box. I have one of those too. Somewhere in a storage tote. lol. Big bulky thing. The link didn't post so I guess it wasn't allowed but you have me on the right track so I'll find it.
