Not a perf error, stamp with remaining perfs ('hanging chads') are simply a sign of a incomplete perforation strike. Interesting but not uncommon.

Hanging chads? Perforation holes which are not fully punched-out are fairly common in this era.
If you are referring to something else about this stamp, you will have to be more specific.
