Just found this old thread and saw stampgals cover on the first page asking about why there was label over label. I will take a guess at this one. I believe that the summation below is quite plausable.

One sees that it is a commercial cover and has a machine cancel from Shinjukjnishiokubo Japan. The first label may have had shinjuku or shinjukuokubo on it. Shinjukuo ward is a commercial and administrative center in Tokyo, Japan.

Later when it was taken to the main P.O. it was discovered that it was underpaid by .20, as indicated by the additional postage stamp and cancellation from Shinjukukita, and it was then that the new second registered label from that location was stuck on top of the old.
