Another thought on this one. Notice that the name Montgomery is written differently on the cover as compared to inside. One part written by a secretary possibly?
Judging my the capital M on the cover and the M in Maulson's signature, I think he addressed the letter while the form was filled out by one of his minions.
....Still more thoughts on this. A google of T. Montgomery in Etobicoke in 1855 turned up Thomas Montgomery, owner of Montgomery's Inn. This is a very famous historical site in Toronto which still stands today.

I've sent a letter to the president of the Etobicoke Historical Society to see if she thinks there may be a link between my letter and the historical Mr. Montgomery.
