Hi everyone, I am new to the forum! Wondering if anyone can help me understand what is considered error and if its not an error then what is it? Any help would be appreciated! Thanks in advance.

Welcome.
It is not an error, just a poorly centered stamp. The US was hard pressed to print revenue stamps of this era and the primary purpose of these stamps was pay the tax. Quality control was not as important as the tax collection.
I have moved your thread into this category because we have some very knowledgeable revenue folks who may be able to give you more info on your item.
