Stamp Smarter has a community project for PO department forms.
https://stampsmarter.org/features/N..._ViewDB.html
Click on 'Search On Form Keyword' and type in insure, it has about 70 different insurance related forms.

Thank you for the link, Don. I must apologize for being unclear, Don, let me clarify:

I am not after a selection of forms. Rather I am after "early" as in early usage of the forms. The one shown in the OP is the first ("one") insured package mailed on the first day that Insurance Mail Service became available when both it and the Parcel Post Service started January 1, 1913.

So other package numbers from that date (or say any January 1913 date) or number 1 or "one" on other 1913-1925 dates is more of what I am after. The "one" or 1 on non-1913 years shows when the count was reset to the beginning at that particular post office.

That said I borrowed the images from there, your link, for here (I don't have scans of my examples) to show where the dates and number can show on the various parts.

Parcelpostguy,

Is there anything on the reverse side of this receipt or form? Not all forms have a number or issue date information especially first ones like you show. A quick scan through our form data base does not show anything simuilar that early. Would appreciat your permissioin and assistance adding this and any other forms related to your area of interest we do not have to our data base project. Post here or send .jpg to me and I will bve glad to do that.

Here is the back. If you read my post, these, now front and back, are from the link posted by Studebaker51 and so credited. There is no "Form" number indicated from nor back. These are very small file sizes. However under the properties tab for the not small file size images, one is 1.85 MB and one is 1.54 MB per my PC. Have not run it on the iMac yet.

Due to the small image size, the back text in summary reads, tie the big one on the package, keep the middle sized one in the post office of mailing and give the small one to the sender.

Now you have enough posts to email, so email me. Eventually I will image my Insurance and C.O.D. complete forms (C.O.D. is a tread for another day). We can also then discuss if there are certain used examples of interest. Now the 1-1-1913 OP item is mine and we can discuss that off the forum in email. Lastly I have the newly created for Insurance, Return Receipts (Specimen copy too) awaiting imaging. There were two, one insurance restricted and the other the basic registered RR but with a second numbering line preceded by Insured below the line for the registered number. These are domestic. Foreign Insured RR came a decade later when it was first introduced for US Parcel Post matter to Canada 1-1-1923.

This is the actual insurance form that went with the parcel. Two days later than your receipt (Jan 3, 1913), but the first insured parcel from Eureka, Nevada.

No form number at first, but later in 1913 it was designated Form 3813.

Thank you for that alanmoll. There are many, many form items from Eureka, NV where they were found to be stored there. It is nice to see an important (well to me anyway) early one. Day one number one is fantastic, but equally informative is the date #1 is actually issued.

What adds to the item you show is that it was sent in the intended direction only to be rerouted back to Eureka and for the care of a different recipient.

Postmasters were advised, if they wished, to be open on 1-1-1913 for as long as needed for expected customer demand. 1-2-1913 was just a regular day as was the 3rd. At some offices 1-2-1913 was the first day Parcel Post Service was available to local customers.

Thank you for that detailed information!
I have a six-frame exhibit on U.S. Vended Postal Insurance (1965-1985), Scott QI1 - QI5. This Eureka item is shown as one of the precursor items.

I believe we discussed QI material on ebay via ebay messaging. Now I forget if we got out of ebay messaging and into regular email exchanges. I believe I purchased material from you and passed on some items for you to get. Think back and if that rings a bell, hit me on ebay. I will try to recall (find) you ebay ID and message you. That said I am of the belief you are the current sole exhibitor of QI material.

Of course once you hit 50 posts here at SCF, your email ability unlocks.

FYI,
If you click on the Stamp Smarter thumbnail images in the Postal Form database, they will open the full resolution image in a new window. (Click on images below to see at full size)

Parcelpost,
You basically worked around the rules by using the forum to contact someone. Even worse, now alanmoll has followed your lead and broken the rules too. I ought to lock both your accounts, but I am not because I am in great pain and stress right now (waiting for surgery) and I am assuming that it is shortening my patience.
