Is there any chance this is real?

I'm sure that the fact that the perf holes are not in a straight line is a giveaway.

My real question is it possible for a Farwell perf to be high in the middle pair, and low on the other two sides?

I am just learning about private perfs myself. So I googled Farwell perforations and this is what popped up. I will be spending some time reading this.

No way! This is the easiest of all the Farwells to fake. All you have to do is add one hole to a 4B4. Looks like they tried to add them all on this one. Fools. LMAO
