It's what's known as a semi-postal stamp, which is one issued to raise money for a certain cause or charity; that's what the added denomination is for. I'm not exactly sure what this particular stamp was for; the best translation I can find is "Welfare care".
Is this kind of like the New Zealand health stamps? They had a postage revenue marked (eg 1d.) and another value marked for health (eg 1/2 d.). This 1/2 d. or whatever went to the children's health (charity) camps.
It is a semi-postal. The Germans have a long, storied and commendable history of using semi-postals to help fund charity organizations, the Olympics, children's issues, etc. Even the Nazis made good use of semi-postals to fund "approved" organizations.
This issue (Scott B877 - $4 M and U) depicts Gert Frobe. It pays 300 pf (3 Marks) postage and 100 pf goes to the Federal Association of Welfare Work. I'm not sure what they do exactly.
